Initial Care Assessment
This is an opportunity to get to know you and assess your care needs. Together with your family, a Manager will visit you in your home to discuss potential care and support you may need. As part of this Assessment, we will ask you questions about where and what you feel you need help with. To ensure our work can be undertaken safely in your home we will also conduct a basic Home Care Risk Assessment.
The Care Plan and Home Care Risk Assessment will be formally reviewed on a on-going basis to determine whether the care requirements have changed. If we feel the Care Plan needs to have changes, the Carer will inform our Care Coordinator and the necessary adjustments can be made without delay.
